We have streamlined our processes for work injured patients to facilitate recovery and help get workers back to function.

Preparing For Your First Appointment

Please bring the following items to your first appointment: 

  1. A current drivers license or a picture ID.
  2. All previous x-rays or other diagnostic test films. 

It is important that we receive all previous x-ray films and other diagnostic studies. This includes Myelogram with post Myelogram CT scan, MRIs and CT scans. Radiology reports are helpful, but do not replace the actual films.

If we do not receive your previous films or diagnostic studies by your appointment date, we will have to reschedule your appointment.

To ensure that we receive your films by your appointment date, we suggest that you arrange to bring any previous films with you, rather than having them transferred or delivered for you.  

Additional Items To Bring

 Please bring your completed new patient form along with all applicable items listed below.

  1. Referral(s) and pre-authorization(s)
  2. Previous x-rays, MRI scans, or CT scans: films and reports must be provided
  3. Medical records of previous testing or treatment (EMGs, physical therapy, etc.)
  4. Any brace or support that you are currently or have recently used
  5. Insurance card and applicable co-payment 

Please note, if we have not received all of your relevant medical records and films at the time of your scheduled appointment, your appointment may need to be rescheduled.

Worker's Compensation Requirements

If you are off work, qualify for Worker's Compensation and are being treated for work related injuries, you are expected to keep all your scheduled appointments.

Appointment Cancellations:

Please call 24 hours in advance to cancel an appointment. It is your responsibility to contact your Worker's  Compensation Carrier to let them know of an appointment cancellation.

Medication Refill Policy:

For the safety of our patients, no medication refills can be granted after 4:30pm or on weekends. All requests for refills on medication can only accepted and processed during office hours.
